I think those of you who say this guy doesn't look better are tripping a bit. I know from experience, the worst thing is to have noticeable plugs. And he doesn't have that anymore.

I don't think he looks that odd. Remember, this is a site where some of us spend way too much time fretting over hair (styles, quantity, quality, etc.). This cat is out in public where people aren't thinking "That's an odd pattern on top of his head....."

I would rather have a receding hairline only instead of a bald crown, but a very thin frontal third usually doesn't look good either. Luckily, I'm a diffuse thinner so I don't have to worry....

As long as the patient is happy thats all that matters.
Get use to seeing a TON of Armani guys in 10 years looking just like this fellow except with 5 times the density in the exact same location.
Another procedure and hell look alot better.
He could have lasered the plugs but obviously he wanted some hair .
Work looks solid especially for what you had to work with.

Thanks Dr. Lindsey for your comments on this difficult procedure. I agree that if the patient is happy, that is all that matters. I'm sure he's thrilled to get rid of the pluggy appearance.

One comment on lasering of hair plugs. An office I worked at for 5 years ran 2 lasers constantly for laser hair reduction and did an occasional plug patient. The problem was that even after the hair was gone, and with 4 treatments usually the hair was gone (assuming dark hair and light skin), that the plug skin looked DIFFERENT from the other scalp skin and was still at least as noticable as a scar excision to me. Essentially the plug has become a pencil eraser sized full thickness skin graft. The tissues don't match perfectly, and often times there is just a bit of unevenness to the skin level, and THAT unevenness causes light to shadow in the "valley" on the side of the plug, making it more noticable. (Exactly why I tried to rarely use skin grafts for facial reconstruction after cancer)

Same problem occurs with ice pick acne scars casting shadows. There is a Bryan Adams video from the late '80s where his skin looks fine(you may recall that in most of his videos he had severe acne scars). If you look at the video, almost all of it is shot with lots of direct lighting so that there are no shadows.

Lastly you might wonder why I noticed this, but I referenced it in a Laser Presentation that I used to give to the residents that I taught.

So lasering can be useful for guys like this, but its far from perfect.

Tricky case; perhaps, more than usual, getting the patient into a spot that *he* is most content w/ is singularly important. Personally, I think I would have gone the same route; this guy has been through the ringer, and simply having a nice, natural hairline must feel like such a luxury for him.

Plus, he has a very viable option of using a piece for the rest of his mop.

Guys

Looking at the before picture and armed with the knowledge that the donor supply was limited, I am sure the patient is delighted with the results.

I know the comments above were from respected, veteran posters but whether you or I would "just shave", although a valid opinion, is largely immaterial in judging the quality of the work.

Out of respect, I think the language used should be chosen carefully when expressing opinions of what we would or wouldn't do if faced with similar circumstances.

Given the previous surgery and visibility of the plugs, I think this guy has every right to be extremely pleased with the outcome. He now looks like a guy that has experienced hair loss instead of a guy who has little hair at all and had been scared by previous surgery.

The framing of the face that this result has achieved should not be underestimated.

Dr Lindsey should be thanked and encouraged for posting these types of procedures, it allows for a wider body of results to be viewed and options to be considered by a larger spectrum of forum users.

You know, at meetings, doctors often show only their best results; same is true in ads. Fortunately forums like this allow us to post with no cost and honest doctors will post an entire spectrum of patients and results.

In our office we have B and A pics showing the entire spectrum of patients. It is deceiving to show only model-esque patients with home-run results from fairly small cases. We provide services to patients who are so attractive that they probably could get by without any help from us(or any doctor). But more commonly we offer procedures to average folks who, like me, need a bit of help.

So I agree with your comment that posting cases like this one allows for a wider body of results for the potential consumer.

Thank you Dr. Lindsey, not only for taking on cases like these (of which I am one) but also for POSTING results.

It is nice to see repair cases on this forum, there are plenty of doctors willing to showcase clients who with loads of new hair but the real work in the trenches is taking on cases like these.

I think he looks great and am 100% sure he is much happier than before with the way he looks
