Dr. Meshkin,
Thanks for posting these. At such an advanced norwood stage (he almost looks like a norwood 7), can you tell us this particular patients goals, why he didn't opt for a larger session, etc? 2500 grafts should give him a nice start. Hello Bill,
To answer your question, some people have limited donor and very tight donor areas and if I try to remove wider and larger donor and may run into problem of creating wider scar. There are alot of clinics that push to remove donor hair and do 5000 grafts for every patient, however they donot share their pictures from the scar formation in the donor area after the surgery. Also not everyone can afford to pay for 5000 grafts they are limited in the amount of money they want to spend for their surgery. If they have the money and I can remove safely without damaging the donor area, I will do the megasession. but I never comprise the donor area to get more grafts. |

Thanks for the explanation on your general philosophy. I realize that every patient has different characteristics (donor density, elasticity, safe zone, etc) that a surgeon has to work with that will impact the end result. My questions however, were specific to this patient's particular situation rather than a general philosophy. Clearly there are a few surgeons doing larger megasessions when appropriate for the patient with great success, some of which are exceeding, 4000 and even 5000 grafts. This is certainly an exception rather than the rule in most cases, but the cases I have seen still show very thin fine scars. Again, as you said, it's about the individual patient. So in this patient's situation, was he a tight donor case or only wanted to start with 2500 grafts due to a lack of funds? This patient had limited donor and tight scalp. However we were able to cover reasonable area considering we only did 2500 grafts. Patients' expectations were realistic and we may be able to do one more session to either fill in the front or perhaps work some in the crown area. Usually for patient that have tight scalps, I recommend to start stretch exercise in the donor area two months after the surgery, in case the patient decides to do another session in 6 to 12 months.
