This fellow had a fairly wide hypertrophic scar/keloid from previous work by a large national hair chain. We did a 2 stage trichophytic scar excision and postop steroid injections, and a couple of hundred singles to his hairline (fronto-temporal) at the second scar procedure. He has had no evidence of recurrence and is now discharged from care, with instructions to call me ASAP if any scar issues.
I figure that I will catch some grief about his current scar pics, but this is without a flash, as with a flash, the surrounding hair washes out the scar. Sorry to all of the photo experts out there in advance. BUT, it does show that we took his wide scar and corrected it nicely. Dr. Lindsey McLean VA

Excellent hair line, and it looks like he's going to see a huge improvement in the scar. I have two questions, if you don't mind:
1. If I was interested in just scar revision via excision, would this waste a significant amount of grafts, or do you usually allocate whatever donor is taken out in any way possible? 2. Is this advisable if another strip procedure or two might be necessary in the future? I guess it depends on the scar size, but I wanted to know if it's feasible to undergo scar excision without compromising laxity/donor for future procedures. I might want to improve my scar, but I'm 27 and technically a NW2, so I don't know if it's too early to look into that. Thanks for posting.

good questions: first, it depends on how much hair you have in your scar. this fellow had not much in scar tissue about as tough as you can find. on the other hand, recent plug repair cases that I posted show that we try to get all of the hair around the old plug donor scars, and put them in the recipient site. so....it varies upon the scar. second, i can't predict the future. if i could, i would have sold my citibank stock 2 years ago. so who knows how you will turn out as far as your hair goes....its all genes and luck. just don't try to lower your hairline too much at your young age, or you may regret it. so while i haven't seen your picture, i might be inclined to just do the scar. a poster on this site has contacted me with what sounds like a similar story but a much worse scar. we are likely going to deal with the scar first, as it may take 2 or even 3 cases; and then conceivably deal with hair. solve one problem (the scar) before starting another. or you may wind up with a worse scar, and not enough hair. I added a pic from last week when he walked in. It is attempting to show hair growing through the scar from the trichophytic closure.
The patient is a huge gym guy and seems to have no bad scar issue from continual exercise routines. Dr. Lindsey McLean VA

I may be wrong, but if this guy hasn't stretched in the last 8 months or so, particularly given his workout routines; I don't think he will.
Regarding FUE on him in particular: 1. Look at that tough keloid that I excised initially. I don't think anything would grow in it. It was tough dense avascular scar tissue. 2. He has such FANTASTIC hair surrounding his scar, that unless he chooses a very short cut, noone will ever see the scar he currently has, and probably wouldn't have seen that keloid either. FUE into a wide, discolored scar is one thing, placing grafts into "tire rubber" of a keloid like he had is another. But time will tell. He pops in fairly frequently so I expect that I will know if there are problems. Dr. Lindsey McLean VA
