32 years old patient, FUE Technique, 2 surgeries.
He did not want a drastic change after the first surgery. He preferred to have a better density on the balding area but did not want to lower the hair line nor to cover the vertex. We have pictures of the donor area sent by the patient after 12 days, the zone is very clean and it is unnoticeable to see that he had a hair transplant. First surgery: DAY 1, 1073 grafts. DAY 2, 920 grafts. Then, only 6 months later, the results are already very good. The second surgery was realised after a short delay as the recipient area was not the same as on the first surgery. We worked on the vertex and we lowered the hair line. Second surgery: DAY 1, 1080 grafts. DAY 2, 831 grafts. At the end of the surgery we calculated the total number of grafts: 3904 grafts. That means a total of about 9369 hair. (2.4 hair per graft). The grafts were placed in three types of incisions, of different sizes: 0.7, 0.95 and 1.2 mm. We will add more pictures of this patient in the next few months so to follow the evolution of the surgeries, so an improvement of the results of both the first and the second surgery. So far so good. The patient sounds like he knew what he wanted and went for area coverage as opposed to density. He can always use concealers to thicken up the appearance if he wanted.
How come you used three different sized instruments on him? And what kind of yield do you think this patient got? Last question: How much per FUE graft? Regards, Petchski

Hi everybody. I would like to thank Petchski, my colleague Dr Alan Feller, Please grow please and Bill for their message of encouragement. It's very nice to be appreciated by experts and connoisseurs.
To answer the questions of Petchski. How come you used three different sized instruments on him I 've used three different kind of blades to make the incisions (0.7, 0.95 and 1.2 mm) For the Single hair, we've cut the graft exactly the same way than for a FUT. We had two populations of single hair. The first part was coming from the lower part of the donor area around the neck (pictures 28 and 33 in the first post http://hair-restoration-info.c...21087683/m/665100961 ). It was either one hair single graft or double hair graft. The diameter of the punch was 0.8mm The second part was the follicles transected transformed to have singles hair grafts. Each graft was trimmed and the upper size of the graft was small enough to fit 0.7 mm incision. For the two-hairs graft, I've used a home made punch 0.8mm and they was fit for 0.95 mm holes. For the biggest one, I've used a home made 0.95 mm punch. They was fit for 1.2 mm incisions. You have to know that the follicle is most of the time cone-shaped. And with FUE graft the bottom of the hair is naked, without any tissue around the hair shaft. I leave my main assistant (who will have to place the grafts ) choosing the diameter of the blade and the deep of it. We always try to place different kind of grafts in the beginning of the incision's process to avoid any mistake. We've tend to increase a little bit the size of the incisions with FUE graft. The reason is that the placing has to be very smooth and refined. If the incision is to small the risk of injury during the placement could be extensive. ? And what kind of yield do you think this patient got? As I wrote, we've reach in two surgeries a total of 9369 hairs. The diameter was quite thin. The average size was 63 microns. So the total hair area transplanted is 9369 * (3.1416*0.0315*0.0315)mm2 = 29 mm2 !!! less than a third of one square cm. In my previous post(http://www.hairrestorationnetwork.co...d.php?t=142868) I 've made a mistake. I wrote 'The surface of skin covered by the hair, if we imagine that we merge all the grafted hair together like a ponytail is thus only 0,053 * 0.053 * 3.1416 = 76 mm2' but the calculation was false (sorry for that). In fact it's radius*radius*3.1416*number of hairs. So the hair mass transplanted was only (0.0265*0.0265*3.1416*8689)=19.15 mm2 (1/5 of 1 cm2) and not 76 mm2. So it's fascinating to notice that we obtain such an apparent density with a so small area of hair! For this patient, the result visible after 6 months illustrate the growing of 29/2=around 15 mm2 of hairs. Last question: How much per FUE graft? 6 Euros per graft for the first 500, 5 euros per graft after 500. To remind, the first part of the post is http://hair-restoration-info.c...21087683/m/665100961
