During the last ISHRS (International Society of Hair Restoration Surgery) 17th Annual Meeting, July 2009 at Amsterdam,
I had the honour to be in charge of the ?« Live Patient Viewing ?» as chairman.
About 30 post-op results were presented by a dozen of specialists on those days.
I also invited 3 patients to this Congress, so I had the opportunity to follow and to show the evolution of their surgery.

Among them, I was really glad to welcome this patient.

We planned a second surgery so to increase the density on the vertex, and hopefully we will reach a great result.
Most of doctors (Shapiro, Feller, Marzolla, Ferudini and many others) had a look at his regrowth so to compare the result with the pre-operatory pictures. They were really happy for the patient.

Interesting case, on this patient a total work was carried out in only one stage.

36 out of 50 huge plugs, grafted on the vertex about 20 years ago, were completely removed thanks to a 3 mm punch. Then, I recovered those grafts, and they were replaced. One stitch was enough to close the holes made by the punches.
Laxity was excellent. The strip measured 32 cm of length for an average width of 1.4 cm, its maximal larger was in the middle, 2 cm. After the removal, the actual surface of the strip was 45.5 cm??. For an average density of 80 follicles per cm2 the estimation was to reach about 3650 grafts.

At the end of the surgery we calculated the number of grafts: 3635 grafts.

Singles: 618
Doubles: 1497
Triples: 1187
4 hair grafts: 409
5 hair grafts: 12

That means a total of 8689 hair

The grafts were placed in three types of incisions, of different sizes: 0.7, 0.95 and 1.2 mm.

They were spread in the following way:

zone A 51.6 cm?? received an average of 40 FU/cm??, so 2064 grafts.
zone B 15.9 cm?? received an average of 35 FU/cm??, so 557 grafts.
zone C 50.5 cm?? received an average of 20 FU/cm??, so 1010 grafts.

Even if it seems paradoxical, we will have a higher density on zone B.
In fact, the best grafts with 4 or 5 hair were exclusively used, which gives a total of about 2000 hair for that area only.

The average diameter of the hair is 53 microns, the extreme values are 44 and 62 microns.

The surface of skin covered by the hair, if we imagine that we merge all the grafted hair together like a ponytail is thus only 0,053 * 0.053 * 3.1416 = 76 mm2, which means less than one cm??.

I think that this number is the most important value, so we should consider this factor when we judge the result of a hair transplant.
In fact, it includes the number of grafts, the average number of hair per grafts and the average diameter of the hair.

The name of the software which helps me to measure precisely the grafted areas and the strip, from pictures on which there is a graduated ruler is Hairmes. I personally asked Mr Alexandre Duvallet to develop it. In Montreal, I obtained the price of the second best poster thanks to a presentation on this application, which permits to be much more rigorous and precise on the analysis of the work to be done.


The patient came back to my clinic after 15 days for the removal of the stitches. The suture was excellent, on the vertex as well. This patient made a blog on Hair Transplant Network, do not hesitate to go and have a look at it.

Thanks for an excellent write-up and photo presentation on this patient. Your work is always very clean and very refined. I expect this will turn out to be another excellent result.

I really liked the deep analysis on how the grafts are spread out, it makes for a better perspective.

Another great write up Dr. Devroye. Im really interested in this case. With the 3500 grafts used over a larger area of the scalp, your descriptive breakdown is very informative, it will be interesting to see the end result!!!

*Excellent* work, and it's truly a pleasure to go through your documentation.

I notice that you shave your patients -- do you basically do this for all cases, irrespective of graft count? Also, if you wouldn't mind sharing a brief synopsis your pros/cons for doing so.
