Patient in his early 50's came in to follow-up with Dr. True after his 3rd FUT - follicular units transplanted consisted of 2724 singles, 3781 twos, and 850 threes. His interests were to cover as much of the balding area as possible so the density was uniformally allocated. It is more typical to concentrate the density in the front and top to gain the most cosmetic impact but this depends on the patient's goals and what he/she brings to the table donorwise. He may do a touch up via FUE into select areas.

Wow, this is truly a very impressive result.

I have a question that this post has aroused. The patient has used 7000+ grafts. Im guessing this is most of his donor supply. But the comment that he may touch up with FUE. I know that I should know this but have never really thought about it this way before. FUE is often taken from a wider range of area I know, but how is the limit of FUE calculated after all donor supply is eliminated? Hope this doesnt take this point of topic. Again, the result is pretty spectacular!

Hey Raph.....thanks for the comment....it wouldn't behoove the patient to do another strip at this stage...he actually has viable donor hair between the nape area(closer to the neck) and where his donor scar lies so this is where Dr. True will likely harvest via fue among other spots, difficult to pinpoint the limit on fue but this patient likely has several fue sessions in him before that becomes a concern. the reality is that we're really just doing some tweaking and density fill so the sessions will be somewhat modest.

I want to respond to Raphael84's question.

I have come to the view that one of the most valuable applications of FUE is for patients who have had extensive strip FUT harvesting. In most cases and with most surgeons. Donor strips are taken from the middle of what is deemed to be the "safe" permanent donor hair zone. At the end of a traditional course of FUT sessions, the hair in the temple regions and lower portion of the donor zone is relatively untouched and the thickest remnant of the donor.

Generally in younger patients harvesting from the lower back of the head or "nape" is avoided because some patients with onset of male pattern baldness early in adult life will experience retrograde alopecia in which they lose the hair in this area as part of their balding pattern, thus hair transplanted from this area may not indeed be permanent. (the same consideration must be made in not taking donor hair too high in the donor zone -particularly for patients in their 20's and 30's).

Men in their 40's and older are much less likely to experience retrograde alopecia and it is also easier to predict the true top border of their donor zone. This is the siuation in which FUE from these areas is a great appoach. While it wouldn't look good to take hair in bulk from these areas with strip harvesting, selective thinning by FUE is safe and leaves imperceptible evidence of harvesting.

As a general rule it is possible to harvest 1500 to 2000 grafts by FUE from these areas in a series of sessions. This is a valuable resource for touch ups over the long term once primary restoration has been achieved.

I always emphasize, particularly to younger patients, to not be in a hurry to use up all of your donor supply. Even with faithful use of finasteride and minoxidil there is almost always progression over time, and it is best to leave a reserve for what could be some very important touch-ups. FUE is a great way to address these needs.
