This patient has had a previous HT with large grafts that resulted in a pluggy appearance. He also had a previous and - unsuccessful attempt at correcting the hairline by simply placing small grafts in front of the pluggy hairline. This approach did not improve the appearance of his hairline and is shown as the ???before??? photo. The result shown is following a single session in our office with plug reduction and recycling of the plug hairs as well as additional ultra refined follicular unit grafting.
These images illustrate the importance of reducing the plugs themselves in addition to follicular unit grafting when attempting to correct a pluggy hairline. Simply put, the heart of the problem is the plugs and the focus of the solution has to be reducing the plugs themselves. Dr. Vogel,
I know there are a few approaches to hair transplant repair, in particular, creating a more natural looking hairline. While some physicians choose to transplant hair in between and around the larger grafts, some choose to extract and redistribute the larger grafts. From what I've seen, not only does the approach of each physician vary, but a physician's approach varies according to the patient's circumstances, size of the grafts, etc. In my opinion, this is a very nice improvement.
