This patient had one session of 3691 grafts. These "after" photos were taken about 5 months post-op.

~3700 is a lot of grafts. i hope it turns out to be great and that the gentleman is happy in the end. ~5 months is still way too early for anything constructive to say.
any immediate post-up pics?

I do not have any post op photos on this case. I get many patients wanting to know what the hair will look like at 3- 6 months. So this is a nice example of how much growth is already seen at 5 months, and how the hairline has filled in nicely already. He is a professional DJ on KOOL 105.5 and already he has had numerous comments on his hair. At 12 months it will be even better. Will keep you posted.

Dr Mejia , The patient looks like the growth will be really nice .
How do you approach placement on patients with big cm2 areas to be covered or with naturally big skulls . Keep posting results it looks good .

Very see thru but for only 5 months this is what we should expect. Hopefully he will have some maturation over the next 6-8 months.

every now and then we get to see few prematured ones like this one without immediate post-op shots.
not quite sure why though, besides better something than nothing i guess.
i hope doc will not forget to update this case at the one year mark whether it be good, bad or so-so result.

Thanks for presenting these. However, I do encourage you to come back to update the album with the final results when they grow in.

This should be a good indication to our members that 5 months is way too early to evaluate the results. As you can see, there are several new signs of growth, but nothing has matured yet and it still appears see-through. This is typical for such an early stage.

I look forward to seeing the end result. 3690 follicular unit grafts should produce a very nice result.

OMY: In answer to your question

Graft placement in big skulls varies depending on the individual patient desires and plan, age of the patient and donor limitations. Every patient requires an in depth consultation to determine what their objectives are, what they are looking for and what they do not. Realistic expectations must be given and a customized plan that works for the patient. Graft placement will also vary depending on the shape of the individuals head whether round or elongated.
For example if you look at the patient below, he has a larger head, poor donor density but a desire to get coverage and density. There are many on the online community that have different definitions of density. What is most important is: will the density that you give the patient for the desired coverage make him happy. While we get bogged in the details of number of grafts per cm squared as a definition, the cosmetic and illusion of density is most important in patients with big skulls. Consequently, conservative hairlines are key to minimize the overall square centimeter area. It must be age appropriate and look good for the patient.
It is also important in elderly patients with grey hairs that it is almost impossible to get the total number of grafts in one procedure. Consequently, patients with big skulls, need to understand that to create the best coverage and density, we would need to move as many grafts as feasible over time, whether one, two or three procedures. This can hold true with any patient with a big skull that desires maximum coverage and density.
In the example below, the game plan with the patient was to provide as much coverage as possible to give a mature receding hairline. Grafts placement is concentrated in many cases in the frontal forelock to provide greater density. , In short, I place the grafts in the area that will give the patient the best coverage and illusion of density for the shape of his head and the framing of his head. This is more of a customized on hands visual artistic approach rather than a mathematical formula that is generalized to all patients.
