Sometimes, I think some surgeons decide to have a contest on who can perform the worst hair transplant.
Here is tan example of a 38 year-old French patient. He had two hair transplants in Tunisia, made by Dr A??¦ in Tunis. The doctor,an ex resident of the 'H??pitaux de Paris ?» portrayed himself as a ?« Specialist in hair transplants since 1994?».
The patient had two different procedures and the final result turned out to be a real catastrophe: a hairline where large bouquets hairs were drawn up backwards??¦ and as special gift, two huge scars on the donor area. The patient also contracted a staphylococci infection which left scar after-effects on the donor area.

In our initial meeting and during the consultation I was not far from thinking that the situation is desperate. I considered a two part solution. First of all, it was necessary to remove large number of huge and unnatural grafts with micro punches, especially the ones on the anterior line and particularly those whose orientation was really unnatural. Then we had to decide either, to carry out a FUE, or to make a FUT. Finally we decided to make a FUT for two reasons. First, the laxity of the donor area was surprisingly good despite the fact that the patient had suffered two interventions, and because of the infections after-effects. Then, because of the scar which was really bad, it seemed logical to remove a new strip in order to reduce it.
Thanks to the very good elasticity, I could almost remove the whole scar, and the number of grafts reached was enough to cover several balding areas: from 1 to 4!
1848 grafts including 549 singles, 882 FU2, 360 FU3, 57 FU4 for a total of 3621 hair.

The FU1 (singles) were used to lower the anterior hairline of about 1 cm, and I spread them between some old bad grafts that had not been removed. Most of the 360 FU3 and the 57 FU4 were placed on the central area to give more density to the frontal core.. The 877 FU2 were transfered to the sides and placed in order to get to zone 4. The pictures of the day of the intervention were taken at our clinic. All the other pictures post-op were taken by the patient. He regularly send us pictures.
The evolution of the surgery is excellent, I do not have any pictures of the scar but I know that it is not perfect, it widened more or less about 1 cm.
We are thinking about making a correction to in a new intervention: this time FUE exclusively. I will remove the large grafts still visible, and maybe (if it is possible) I will try to increase a little bit more the density and to repair the scar by placing grafts on it.

Some remarks on the pictures:
Two series of pictures show the work completed on the left side of the hair front line, where the situation was the worst (large grafts removed via a punch of 1.3mm).
The grafts which were more or less correctly orientated, and in the middle of the head were not removed and I surrounded them with single or FU2.
The 6 photographs taken chin down, show that the former anterior line is slowly vanishing as it is gradually being hidden by the new hair.
The situation after 124 days shows precisely how the old grafts are vanishing, and how the frontal line doesn't seem anymore dense and now the central core is more relevant.
The situation at day 148 is very promising.

Once again Dr. Devroye the work looks outstanding. Equally as important though is your thorough explanation of what was going on with the patient and detailed photos showing all of the different zones. I really enjoy when you post because it is so informative. Also, very nice coverage for only 1848 grafts and such a short amount of time. Impressive.

Damn. Someone chopped up this guy read good.

Really good that you fixed him. It looks pretty much natural now althought thin. Is the patient coming in for a 2nd surgery?

Fascinating story and presentation. Thanks for sharing. Keep up the great work.

Your graphics suggest to me that there is a real need for a CAD tool in HT procedures so that patients and doctors can plan optimum strategies for their HT. Such a tool would input several 2D images, creates a 3d model and then allow the Dr to manipulate the graft distribution to suit the patients need. A growth feature would then simulate growth and maturity for a virtual "final" result.

This is an example of solid repair work. Your documentation of the case is also excellent. I would be interested in seeing the future progess of this case.

Dr. Devroye,

I must first commend you for your photo presentation. High-Res photos, comprehensive details including "Day 0", "Day 1", zone 1, single hairs, etc. - it's all definitely to be commended!

As for the repair, again, I must commend you for your planning and what appears to be a very successful repair at this early stage. I look forward to further updates.

This should be used for reference when new people come on and ask "What's the best way to remove old plugs?" b/c this shows the punch out method, which I prefer, and shows how undetectable it can be after a few months.

Great job for this fellow.
