This patient looks pretty young but he's actually 31 years old. For him, the receding look bothered him a great deal. He no longer feels like a balding man and is thrilled with his results.
I think the fact that only 1500 grafts used for the hairline indicates Dr. Paul used appropriate # of grafts taking future loss into consideration and reserving donor hairs to address future loss, if it occurs. We at SMG try to educate our patients as much as possible so patients go into hair restoration surgery with their eyes wide open. So even when a prospective patients calls and says "just sign me up, cuz I know everything", they are given a thorough consultation each and every time.

I have to say this actually had a very nice cosmetic impact...the hairline was not lowered dramatically but is age appropriate. When you look at the temporal area you see how it has been rebuilt with peaks and the gaps in the corners where there were signs of separation have been closed. Well planned.

Very valid points, I wish more were cognizant of the importance of this and appreciated doctors/ clinics that keep this in mind. It is critical to never losing sight of future progression and to assure that the work that is created will looks appropriate now and down the road with future thinning.
In keeping this patients hairline more or less where it is and just re-establishing it so the patient would not see through it as much, along with providing a solid improvement in relation to where he was and allowing him to be on top of his problem, was the goal. We were not re- establish his 18 year old hairline that would be low, broad, and strong and would sacrifice a lot of hair, early in this patient progression, which would or could be very inappropriate. We keep all these factors in mind. We used no more hair then we had too to meet the patient's goals, placing the hair line in an area that will look natural now and down the road, even with extensive hair loss. In following the patient, if we find he bucks the system and does not have extensive hair loss, we can always lower it if there is a need. Ultimately, hair lines are in dead sync with how much hair is behind them, if one has a low, broad and strong hairline they better keep a full head a hair behind it, otherwise it could look peculiar and the young guys thinning are the least likely to be able to maintain this. We understand why many young guys want low, broad, strong hair lines, it make sense??¦.they want what their use to having, they want what their peers have, they are not usually set professionally and socially and they want to be at their A game. And, there is nothing more we would love to give them, unless it could hurt them in 10-20years. If we do something now that makes them happy but in 20 years their backed into a corner, we done them no favors. Actually, and it is unfortunate, the young guy is really the last guy to want to create and low broad strong hairline on, even though they are the ones that want it the most. These are the patients that generally possess genetically the great potential for future hair loss, in that they started thinning young. The younger one starts thinning usually the more extensive the hair loss is, this is the tendency and because we do not have a crystal ball and we are dealing with a moving field and limited source we prepare for this (extensive future progression) until something tells us otherwise. All and all, doctors should be very cautious in being to aggressive on young men both in the very anterior and posterior regions of the male pattern area.

Bob C,
Interesting points. I think deciding to undergo hair transplant surgery is a very personal decision. Whereas this may not be a "wow" transformation due to the fact that he already had plenty of natural hair, clearly his hair loss was bothering him enough to get work done. I do agree there's been some cosmetic impact. Since 1500 grafts is a smaller session and in the event of future hair loss, he'll have plenty of donor hair for future work. ] Janna, Is this patient on Propecia to help prevent future loss? Also, how old is he? Nice work. Bill

I've added a frontal shot of before and after, which was mismarked.
This patient is indeed on Propecia and he's 31 years old.
