This man in his mid 30s has balding in a Norwood 3A pattern but has early thinning throughout on top. He has average diameter, black, wavy donor hair. He had more single-haired grafts than average in that 537 of 2644 grafts (20.3%) were natural 1s.
I originally offered to transplant 3000 grafts in front but 2500 grafts fit his budget better. We therefore planned to transplant 2500 grafts a little less densely at 56 fug per cm2 in the midfrontal forelock, 48 behind his hairline and 42 in the rest of the frontal area. His donor strip averaged 1 cm in width and he had a trichophytic top edge closure with moderate undermining of both edges and a single-layer of Prolene sutures. (Since then, I have switched to a lower ledge trichophytic technique as it seems to work a little better for me.) He started Finasteride just after his consultation, 13 months ago, and his crown does look better. He chose not to use Minoxidil. He is quite happy with his results but may wish to fill in the front a bit more later. I recommended against lowering his hairline or filling his FT recessions more as it could look unnatural after he got older, especially if he lost a lot more hair. He is still young and still has a very large area of potential hair loss. While he plans to continue taking Finasteride, we can't count on him taking it forever. His chosen hairstyle helps make his transplant look thicker so he may leave things as they are for now. I invited him to join the forum so he may sign up and ask your opinion.
