DR Ibrahim a doctor by profession was type II ish was interested to have a very dense looks. His hair quality was very thick and most of the follicular units wee two three and some were even 4 hair graft units, this can be a problem to make a soft hair line. SO, the front was dneby splitting few grafts into ones and making the hair line softer.
He had only 1448 grafts on 24th August 2008 and post op pictures were taken on 7th of May 2009. so only 8 months post operative results so far.

You will appreciate the two and three hair follicular units when you see the donor area tricophytic scar as well. He has very think hair shafts and more hair per follicular units. This can some times give you a pluggy looks inspite of using the follicuar units.
I do two things to make it look softer and non pluggy,
1) split the follicuar units for the first 1 cm of transition area.
2) if that does not work then do a laser hair removal 1 or 2 sessions on the hair line, I showed this back in 2003 ISHRS meeting in NY, discussion, and then in 2006 San Dieago ISHRS meeting when I was moderating the session for ethnic hair transplant.
Mind you the density is not more than 35/cm. looks pretty ok to me and the patient.

Hair transplant is not about density but about looking normal and acceptable density.

Your right , it does look pluggy.
Maybe he can try nape hair or leg hair via fue .
Or a simple combed forward look

I think in this case f you see, the first impression is not pluggy at all but very dense and yes ofcourse if you look very close it does have few elements of being more think than usual.
He is quiet happy with that and have not complained so far, but that option of laser is always there.

What I do with laser is not hair removal its just to thin it out. Any Hair removal laser will do the trick and all you need to do is just shine the laser once or twice on the area of pluggy look or hard hairs and then review in 3 to 6 months, and asses the thinnes if ok than thats all you need.
