FUE - 1397 grafts 2148 hairs - Dr. Paul Shapiro

I am pleased to present a 2 Day FUE session with Dr. Paul Shapiro.

This patient is a local patient (age 28) who wanted his hairline restored via FUE procedure. It's a two day session totalling 1397 grafts. The goal of 1200-1400 grafts was achieved successfully. The procedure took place Mar 3rd and 4th.

1st day ??“ 820total??¦.361- 1 hairs, 458 two hairs and 1-3 hairs

2nd day- 577total??¦286-1 hairs, 291 2 hairs

Janna,

It's nice to see some of Dr. Paul's FUE work. How is Dr. Ron and Dr. Paul liking working with FUE? How do they feel regarding its consistency compared to strip in terms of growth yield, candidates, etc?

I'm looking forward to seeing this patient's grown out results.

They both like the FUE method. This patient is a local patient so we're hopeful he'll be by on a regular basis to take progress pictures.

With our careful and conservative approach to fue, we truly don't see why the fue grafts wouldn't be as viable as the strip. We trim and examine each graft under the microscope after each extraction. Since the follicles are more vulnerable via fue because they don't have the protective tissue surrounding it, they need to be planted as soon as possible and more delicately than traditional strip grafts. We also limit our fue session sizes to 1000 grafts or less per day. Keeping all this in mind and taking necessary steps to address all concerns, we don't see why we wouldn't get the same yield with fue as strip method. We are encouraged by our feedback from planting fue grafts into scar repair patients. If the grafts are growing in scar, it stands to reason the fue grafts will grow in healthy skin with our method.

I asked about the punch sizes used on this patient and the punch sizes were .9mm and 1.0mm because the patient has larger than average follicles and he had majority of FU groupings of 2, 3, and 4's. The 3's and 4's were trimmed down to 1's and 2's for the hairline and temples.

Looks really nice Janna.
Ill be honest I wasnt so sure about fue when I came there.
Mainly because of how recently SMG started offering it .
I trust you guys so much I knew you wouldnt offer it if you didnt think Id be happy and Im here to tell you Im ecstatic
Theres been a dramatic change in 4 months
I swear to you I believe every graft has sprouted .
I will definitely go fue for the crown as Im done with strip.
Ill also take some more pics at 6 months and send them.

What exactly makes a candidate to an ideal patient?

The young patient in the attachement has a relation of 1,55 hairs/graft and looks totally natural. Young Hairline Patient - Dr. Ron Shapiro

Other natural results I've seen have been patients with thin hair and a low skin/hair contrast. For example NW5a (Dr. Heitmann) and chicks_dig_guys_with_hair (Dr. Rahal) have very natural hairlines. Both have also fine hair in the donor. I understand that you need multigrafts to create density but every hairline needs up to 800 fine single grafts to copy nature. The thick grafts are always darker and thicker than the native miniaturized hair in the front. The finer grafts are generally brighter and cause the natural appearance.

This is my view, am I right or do I miss important facts???

Hi AJKV,

When I say ideal patient, it's not necessarily based on their hair characteristics. That's only part of the equation. We performed FUE on this patient and it went very well. The extraction was straight forward, the patient has many groupings of 3 and 4 hairs in the donor area, which grow faster than average. His skin texture and hair angle was such that it was considered easy to extract each graft intact without any damage. He is also a local patient who will be able to come into our clinic on a regualr basis to have his pictures taken to follow his progress at every step.

I'd have to disagree with the 800 single hairs needed to copy a natural hairline. From our experience, we generally plant 400-500 singles to create natural looking hairlines.

What does your clinic do to ensure the grafts are not damaged during the extraction process since the procedure itself is blind? In your clinic's experience, what, if anything adds to the risk of transection during the extraction process and what steps has your clinic taken to overcome them. Are there any remaining hurdles?
