I have had 7 emails and calls in the past 2 weeks asking specifically how we do strips and if there is anything special about our technique.

Its fairly verbose and a picture is worth a thousand words, so here are a few pics.

We don't do anything particularly special, but we do handle the strip gently (my hands--not a clamp) and we do skinner strips to facilitate a tension free trichophytic closure.

The case shown was done Friday and will have sutures out Thursday.

Of particular importance it seems, to many questioners, is that we don't shave anything but the skin we are excising. The tape holding up the hair is removed allowing hair to cover the sutures in almost anyone with 1/2 inch long hair or longer.

We do the same technique for second strip surgeries except that we excise the original scar if in an appropriate position. And we do the same technique with MOST scar revisions except that we may undermine the surrounding scalp to decrease closing tension.

I hope this helps those of you who have contacted me recently.

Readers:

In the third sentence....we do skinnier strips-not skinner strips.

Sorry for the typo.

I like longer, thinner strips so that we can have very little tension on the wound edges.

Dr. Lindsey, I really appreciate the time taken to post these pictures. I have wondered what the back of my head would look like during the procedure and after the strip was removed. Now, I know.

Thanks for the education and sharing your insights on many other topics in this forum. I must add - your spelling mistakes are unpardonable! :P

Take care.

Dr Lindsey, do you release the scalp from the cranium on either sides of the closure before closing? What is the common practice?

I just put up his pic with sutures out. Day 6.

No, I don't release tissue in routine cases.

Thanks,

Thank you Dr Lindsey. Generally, is this technique(tissue releasing) used? Any input from clinics would be appreciated.
