Thanks for the feedback. We did not shave the area. When possible, I do like to shave because it makes the surgery go faster. But most of my patients expect to be back to work in a week with minimal detectability. In this case we did lateral slits throughout the hairless area and blended with needle sagital slits into the existing hair so we didn't have to shave.
The comment from the peanut gallery is well taken. I don't undertake hairline restoration for a guy in his mid-20s lightly. We have long discussion about the consequences of having permanent grafts surrounded by hair that may be lost in the future. I wouldn't do this in a patient with miniaturization throughout their scalp or a strong family history of baldness. While some people may think he looked fine beforehand, he was tremendously bothered by his hairline (but not obsessed and he was otherwise psychologically healthy). He was thrilled with his results and for him, the potential for needing further grafts in the future was well worth the benefits of a restored hairline. Doing a relatively conservative number (1500) ensures we have plenty of donor for future use. This would not be the case if 3000 grafts were done. Thanks for the interest in this case.

I have great regard for your work and abilities and this guy's hairline is perfect, a bit too perfect. IMHO, calling this a case of severe fronto temporal recession is severe He looked great b4 - very natural, but now he is too perfect and so I would really prefer his pre-op look. He will never have a mature hairline. I fully respect his feelings and decision but this is my opinion.

His hair could be a little course which is giving the appearance and in turn needs a little "filler" to soften if needed. Pics can always be a little deceptive though and seeing the result in the flesh would give a different view altogether - Any video at all doc as that may help.

Initially, I thought this was an extremely impressive result. And I still do, though I understand the "nit-picking"....for all intents and purposes it looks totally natural, but when pulled back...ya....I can see the imperfection of it, but I don't think this on Dr. Cooley's hands so much as it is the nature of the beast, like Spex has described.
It's obv a very "youthful" hairline, which I'm sure is exactly what the patient desired.

I see what you all are talking about, and fwiw, mine doesn't do that. But, I've got what is usually described as 'baby fine' hair.
