The average density for the entire frontal graft zone was approximately 20 follicular-units per square centimeter. However, the actual density of the right, central and left hairline regions varied based on the specific goals expressed by the patient. His main concern for density enhancement was in the scarred region of the right-central hairline. Density in that area was approximately 35-40 grafts per square centimeter. This transitioned to about 15 grafts per square centimeter in the right frontotemporal recession. A lower density was used in the left frontotemporal recession as that side was of least concern to the patient. Approximately half of the grafts in the scar zone were placed into pre-cut openings and half were inserted using stick and place in order to get a sense of the local circulation in the area. Using too high a density in the severely scarred zone could have risked additional scarring and poor yield. The patient had expectations for high density in the frontal hairline and agreed to a staged plan for maximizing frontal hairline density while keeping the risks at a tolerable level. It was recommended that he wait at least one year before attempting any additional surgery to allow for a full recovery of the circulation within the scarred area.
